Appointment Check List
What to Bring to the Clinic
- Your child's insurance card and co-payment (if the plan requires a co-payment)
- Your family medical history
- Symptoms and timing of any medical conditions (including past conditions)
- The names and doses of your child's medications
- Details of any allergies your child has
- Your questions and concerns
- Your appointment calendar
- A toy for your child to play with while waiting
Before You Leave the Clinic
- Understand what to look for and how to report changes in symptoms
- Ask when your child needs to be seen next and schedule the appointment
- If your child is given medications, understand how much should be given, when, for how long, side effects to watch for and how to handle prescription refill needs
- If your child is referred to another provider, understand what the provider will be evaluating and how the caregivers will partner in treating your child (if your health plan requires a referral, work with your primary care clinic's referral coordinator to obtain the necessary authorization prior to scheduling the appointment)
